What are Perinatal Forms?

The British Columbia Perinatal Health Program (BCPHP) has the provincial mandate to develop a suite of standard perinatal forms used by most health care providers across British Columbia. These forms represent "best practice" in perinatal care as well as act as a documents tool to record patient care. These forms are the source of data for the British Columbia Perinatal Database.

The BCPHP with the assistance of multidisciplinary working committees reviews and revises the forms on a regular basis.

Labour Partogram and Perinatal & Triage and Assessment Record

 

Dear Colleagues:

The BC Perinatal Health Program convened a working group of representatives from Health Authorities in January 2009 to revise several Provincial Perinatal Forms. The revision included the Labour Partogram, a new Perinatal Triage and Assessment Record (formerly Maternal Assessment Record) and Care Pathways for Postpartum and the Newborn.

The purpose of the revision was to ensure that provincial documentation tools are useful and applicable for al levels of maternity care, reflect current evidence-based practice, adopt new terminology for fetal health surveillance, and facilitate documentation for outcome evaluation.

We are pleased that the revision for the Labour Partogram (BCPHP 1590 February 2010) and Perinatal Triage and Assessment Record (BCPHP 1590 February 2010) are now complete. Care providers from eleven hospitals and several midwifery practices participated in a 4-month pilot from June to September 2009, and subsequently provided extensive feedback to improve them.

These forms will be available in mid-March. Any orders received in March will be held until the revised forms are available for circulation. The older versions of the Labour Partogram and Maternal Assessment Record will be retired gradually after March 31, 2010. Please start using the new forms by April 1, 2010.

Individual Practitioner Orders

Individual practitioners may obtain copies of the forms from their local hospital. If an individual office orders copies of the forms from RR Donnelly, pack & freight charges will apply.
